  1. Collaborative Approach

A caregiver's knowledge about the person they’re caring for is vital. We work hand-in-hand with families, caregivers, and the State nurse during this process. As your loved one’s caregiver, your insights and concerns are crucial in developing a Plan of Care that reflects the patient’s unique situation.

We can also consider and integrate your loved one's doctor's recommendations into the care plan, ensuring it aligns with their overall medical treatment. 

During this step, a State-appointed Registered Nurse (RN) will complete their visit and assessment. Sometimes this interview occurs in your home and at other times it may be conducted over the phone.

  1. Free In-Home Comprehensive Consultation

With this step, our program is activated and you start your in-home care plan. Our care coordinators come to your home for a detailed consultation and discuss your loved one’s needs with you in a familiar environment.

During this free consultation, we work with you and your loved one to thoroughly understand their:

  • Medical needs by reviewing their medical history and diagnoses.
  • Acts of daily living (ADLs) they need support with. This can include things like bathing, dressing, eating, and toileting.
  • Functional abilities to assess their mobility, strength, and overall level of independence.
  • Personal preferences for daily routine with valuable inputs from you.
  1. Optimizing Benefits

Our team is well-versed in the SD-CFC/PAS program and Medicaid guidelines. We'll explore all available options to ensure your loved one receives the maximum support they deserve.

  1. Adjusting the Plan of Care
